Thank you for your interest in joining the Ohio State Medical Association! There are two simple ways to join:

  1. Join online today. Start the online application process today by entering the required information below and clicking search. If you locate yourself, follow the onscreen instructions to become a member. If you’re unable to locate yourself, please call our resource center at (800) 766-6762.

  2. Download the application by clicking here. Then either mail your application to 3401 Mill Run Drive, Hilliard, Ohio 43026, or fax it to (614) 527-6763.
