Accreditation Statement for Designated Activities

The standardized statement that must be used on all brochures, course announcements and all other promotional materials that confirms the name of the accredited provider and the accrediting body which recognizes the provider as an accredited provider of continuing medical education offerings. The accreditation statement for intrastate accredited providers should read exactly as follows.

The [name of accredited provider] is accredited by the Ohio State Medical Association to provide continuing medical education for physicians.

Accreditation Statement for Joint Sponsored Activities

AThis activity was planned and implemented in accordance with the Essential Areas and Policies of the Ohio State Medical Association (OSMA) through the joint sponsorship of [name of accredited provider] and [name of non-accredited provider]. The [name of accredited provider] is accredited by the Ohio State Medical Association (OSMA) to provide continuing medical education for physicians.

Accreditation Statement for Co-Sponsored Activities

This activity has been jointly planned and implemented in accordance with the Essential Areas and Policies of the Ohio State Medical Association by <insert name of OSMA accredited provider 1> and <insert name of OSMA accredited provider 2>. The <insert names of OSMA accredited providers> are accredited by the OSMA to provide continuing medical education for physicians.

<Insert name of OSMA accredited provider 1> has been designated to take responsibility for the content, quality, and scientific integrity of this CME activity.

The provider identified as taking responsibility for the content, quality, and scientific integrity of the CME activity will be accountable for ensuring that the activity was planned, implemented, and evaluated in accordance with the Essential Areas, Elements and Policies of CME, and will be responsible for maintaining proper documentation of such.

Note (1/1/01): The ACCME no longer recognizes the co-sponsorship accreditation statement. CME activities that are co-sponsored should use the directly sponsored activity statement, naming the one accredited provider that is responsible for the activity.

Accreditation Statement on All CME Activities (00-03-H)

The accreditation statement must appear on all CME activity materials and brochures distributed by accredited institutions/organizations.

Accreditation Statement - Save the Date (00-03-R)

The accreditation statement does not need to be included on initial, save-the-date type activity announcements. Such announcements contain only general, preliminary information about the activity like date, location, and title. If more specific information is included, like faculty and objectives, the accreditation must be included.
