Retention of Activity Files/Records

Accredited providers are required to maintain activity files/records in accordance with required documentation for compliance with the Essential Areas, Elements & Policies. It is the opinion of the OSMA Focused Task Force on Accreditation that all activity files/records should be maintained for a minimum length of time of the current accreditation period or the last twelve (12) months, whichever is longer.

Retention of Verification of Attendance

In compliance with the State Medical Board of Ohio guidelines for relicensure, physicians are responsible for keeping evidence of their CME activities for six years in the event they are audited by the State Medical Board of Ohio. For this reason, the OSMA Focused Task Force on Accreditation recommends that accredited providers maintain attendance records for a minimum of six (6) years.

99-08-E Documentation by OSMA for Accreditation Process

The OSMA Department of Educational Services maintains a system for the collection, storage, and retrieval of data necessary for the accreditation process. Documentation descriptive of the accreditation review process for each OSMA accredited provider of CME will be maintained until the next accredited decision. Some documentation may be maintained for longer the current accreditation period for the purposes of compiling historical data.

This documentation includes (but is not limited to) the following:

  • Completed application forms from the accredited provider;
  • Relevant correspondence between the OSMA and the accredited provider;
  • Survey team reports;
  • Actions taken by the OSMA Focused Task Force on Accreditation;
  • Communication between the OSMA and providers regarding accreditation decisions;
  • Communications between the OSMA and the ACCME regarding accreditation decisions;
  • Follow-up reports generated by the provider, which include Annual Reports and Progress Report(s), if required.
